The Science of Sugar

Some languages have many different words for love. Some have many different words for snow. One language even has different words for a tasty layered sandwich. Is that a hoagie or a hero you’re having? A sub? Grinder? Po’boy?

“Sugar,” though, is a single word which has taken on many meanings, from casual endearment to monosaccharide molecule. Today, we’re examining scientific definitions, with a short and sweet look at the science of sugar.

Chemistry

First, let’s get basic—all the way down to the molecular level.

Carbohydrates, fats, and proteins are the three essential macronutrients which keep our bodies running. Sugars are molecules made of carbon, hydrogen, and oxygen atoms, and all carbohydrates are made of these sugar molecules, from single unit structures to very complicated chains.

Sugars called monosaccharides are the most basic of the carbohydrates. “Monosaccharide” comes from the Greek words for “single” and “sugar,” and monosaccharides are the sugars we mean when we talk about “simple sugars.” Why are they simple? Because a monosaccharide is a single molecule which can’t be broken down into smaller carbohydrates.

While there are several types of monosaccharides, the three major simple sugars are:

  • Fructose—the sugar we get from fruit.
  • Galactose—the sugar found in milk.
  • Glucose—the sugar our bodies use the most. It’s found in plants, and also produced when our bodies break down other carbs. Fun fact—this is the only sugar essential for brain cell function.

When two monosaccharide molecules join together, they form a disaccharide (not surprisingly, from the Greek for “two sugars”). Again, there are quite a few disaccharides, but we tend to concentrate on three in our diets:

  • Lactose (glucose + galactose)—found only in milk and dairy products.
  • Maltose (glucose + glucose)—the sugar chiefly produced by grains.
  • Sucrose (glucose + fructose)—produced in plants. These plants include sugar cane and sugar beets, from which we get our refined table sugars.

The reason sugar molecules are so important is because of how our bodies use them.

Biology

Our bodies use the glucose in carbs for energy. As foods break down, first through digestion and then in the cells, the chemical bonds which hold glucose molecules together break as well. This action releases energy, and this energy fuels all our bodily functions.

But even though we need carbohydrates to keep our bodies going, and even though sugars are the easiest carbs to use for energy, there’s a reason no one recommends a diet filled with extra sugar.

Our bodies get all the sugar they need from the natural sugar in the foods we eat. Natural sugars are found in fruits, dairy products, even some vegetables and grains. Along with that sugar come vitamins, minerals, antioxidants, fiber, and/or protein.

Extra sugars added during baking or mixing or processing for flavor and sweetness provide none of these nutrients. These sugars are known as “added sugars,” and have more serious consequences than just empty calories.

Once we have the sugars we require, there’s no need for more. Extra sugars are stored in liver, muscle, and fat cells for later use. When we eat too much sugar, this carefully balanced system is upset, with negative effects for, among other things, weight, blood sugar, insulin levels—and our dental health.

Nutrition and Dental Health

You know that a sugar-filled diet often means a cavity-filled checkup. Why? Because it’s not just our bodies that break down sugar for fuel.

The oral bacteria in plaque also need sugars for food, which they use to make acids. An acidic environment in the mouth weakens and dissolves the minerals which keep tooth enamel strong. And these weak spots are vulnerable to decay. A steady diet of sugar-filled foods means that your enamel is constantly under acid attack.

Cutting down on added sugars is one of the easiest and best ways to cut down on added cavities. Reading recipes, checking out labels, learning to recognize added sugars—this is nutritional research which has sweet results.

How to recognize added sugars? Here’s where language gives us plenty of words to fill our linguistic sugar bowl. Whether the ingredients are called agave nectar, honey, molasses, syrups, treacle, or table sugar, they’re really just sugar. More specifically, they’re all sugars made up of glucose and fructose, with at best a trace amount of vitamins and minerals—and usually not even a trace!

To make our lives easier, labels on food packaging now let us know exactly how much of the sugar in any product is “added sugar.” You expect to find a high percentage of sugar on dessert labels, but you might be surprised to read how much sugar is added to foods like energy drinks, sports drinks, flavored yogurts, cereals, spaghetti sauce, and many more of the items in your grocery cart. Spend an extra minute examining the label, and save yourself many empty calories.

Monosaccharides, disaccharides, glucose, fructose, maltose, agave syrup, treacle, and on and on—so many words for so many kinds of sugar. Can Your Dentist Tell If You’re Left-Handed?

Sherlock Holmes, that most famous of fictional detectives, observed clues, analyzed them, and used his powers of deduction to arrive at the solution to his cases.

Dr. DeCasperis and our team are skilled in deduction as well! By examining your teeth and gums during your regular checkup, important clues can be discovered about your dental health—and perhaps your overall health as well. What might be deduced when you come in for an exam?

  • You Haven’t Been Flossing

Even with some last-minute catchup flossing, it’s not hard to tell if you’ve been regularly skipping the flossing portion of your daily dental care. Plaque that your toothbrush just can’t reach on its own will be noticeable between your teeth and around the gum line. Your gums might be inflamed, swollen or bleeding—early signs of the gum disease plaque causes when it’s not removed with careful flossing.

These symptoms could arise because you’re neglecting to floss at least once a day, or it could be that your flossing skills could simply use some fine-tuning. A refresher in flossing technique takes no time at all. Or, you might discuss whether a water flosser would be a good investment for healthier teeth and gums if, for any reason, you have difficulty flossing effectively.

  • You Haven’t Been Sleeping Well

Even if your roommate or partner hasn’t told you that your teeth and jaws have been grinding away while you were not-so-soundly asleep, your dentist might be able to. Shorter, flattened teeth are a common symptom of bruxism, or tooth grinding. Your exam might reveal cracked enamel or broken cusps caused by the force you’re placing on them at night. This condition can also lead to larger jaw muscles and jaw pain.

Since these symptoms are also caused by sleep apnea, it’s a very good idea to find out what’s causing this dental stress. Treatments are available to make your sleep safer for your teeth and jaws—and more restful and healthier for you!

  • You Should Talk to Your Doctor

Conditions that seem to have nothing to do with your oral health can cause dental symptoms, alerting your dentist to a medical condition which you might not be aware of. Pale gums or a swollen tongue could be symptoms of anemia. Erosion on the inside of the teeth can be caused by GERD, or gastroesophageal reflux disease. Vitamin and mineral deficiencies can lead to ulcers, loose teeth, and gum disease.

  • Are You Left-Handed?

So, getting back to our original question, can your dentist deduce that you’re left-handed?

Probably not! It’s been suggested that left-handed people brush their teeth more firmly on the left side, and right-handed people brush harder on the right. And it’s also been suggested that left-handers don’t brush the left side of the mouth as thoroughly, while right-handers neglect their dominant side.

But Dr. DeCasperis can still tell you some important facts about your hand-brush coordination just by observing the state of your enamel. If you’re brushing too firmly, your tooth surfaces could show erosion and wear. A soft-bristled brush and a lighter hand will get your teeth just as clean without the abrasion.

How can parents help prevent tooth decay?

Children are born with a set of primary teeth – 20 to be exact – that help them learn to chew and speak, and develop enough space in the jaw for the permanent teeth that will appear several years later. Kids are especially susceptible to decay, which can cause pain and tooth loss – a problem that could interfere with oral development. As a parent, it is important that you take proactive steps to keep your child’s teeth as healthy as possible.

Bottles and “Sippie Cups”

One of the biggest culprits of childhood tooth decay is poor diet. This begins as early as a few months old, when children are often allowed to go to bed with bottles and “sippie cups” of milk or juice. The sugars in these beverages – even natural sugars – can steadily decay the teeth.

Dr. DeCasperis and our staff suggest serving children milk and juice only at meal times, and limiting juice intake to just a few ounces per day. If your child becomes thirsty between meals or likes to go to bed with a bottle, serve water during these times.

Hygiene

As a parent, you can establish healthy dental habits long before your child’s first tooth erupts. Start by gently wiping your baby’s gums with a clean wash cloth during the first months of life. By age one, graduate to an appropriately sized toothbrush with fluoridated toothpaste, and brush at least twice a day.

Dental Visits

Dental visits should start young and continue on a regular basis throughout your child's life. Heart Disease and Oral Health

Is there a link between oral health and heart health? This is a question that has been receiving a lot of attention in recent studies. While as yet there is no definitive study that proves oral health has an effect on overall heart health, there is also no definitive proof that oral health doesn’t affect heart health. So, what do we know, and, just as important, what should we do?

What do we know? One clear connection exists between gum disease and endocarditis, a rare, but potentially very serious, infection. When we neglect our gums, they bleed more easily. When gum disease advances, the gums pull away from the teeth. This creates pockets which can harbor more bacteria and plaque, leading to infection and even tooth and bone loss. And this bleeding and infection can be serious not only as a matter of dental health, but for your heart as well.

Endocarditis is an infection of the inner lining of the heart and the heart valves. When bacteria from other parts of the body, including your mouth, make their way into the blood stream, they can attach themselves to damaged areas of the heart. The resulting infection can become very serious, very quickly. If you have damaged heart valves or artificial valves, have an artificial heart, or have any type of heart defect, it is especially important to prevent gum disease from developing. Regular dental care will help prevent bleeding gums and infections, and reduce the danger of oral bacteria entering the bloodstream. What else might link oral health and heart disease? Because an increased risk of heart disease has been found in patients with moderate or severe gum disease, there has been much speculation as to a possible cause-and-effect connection. For example, the plaque inside arteries which leads to heart attacks has been found to contain certain strains of periodontal bacteria. Other studies have searched for a causal relationship between inflammation resulting from gum disease and inflammation leading to heart disease.  The body’s immune response to periodontitis and its potential consequences for heart health have also been examined. So far, there has been no clear evidence that treating gum disease prevents the occurrence or recurrence of heart disease.

So what should we do? This is the easy part. Keeping your gums and teeth healthy will increase your quality of life in immeasurable ways. And it’s not difficult!
